SUPER VAR CUP ROSTER REGISTARTION

Roster rules: -Last chance to adjust any rosters is Friday, August 4th by Midnight. - Players will be added to their selected team roster by filling out this form. - Players cannot register for more than one team. - Teams can have a maximum of 14 players. - If a player is not on the roster they will not be allowed to play (no tolerance). - The roster is required to have at least 5 players before the first game. -If a player is reported not to meet any of the rules listed above they will be removed from the game upon confirming with the organizers/referee. The team will also get a warning, if this occurs again then the team will automatically be forfeited the game. -For tournaments, No call ups are allowed to play. Game rules: -Games will be 7v7. - Games will be 25min long. -Each team must bring 2 shirts ( a dark and light color) - Free-kicks will be direct after the half-line (offense) and indirect before the half-line (defense). - If a player receives a yellow card, he shall be sent off for 2 minutes, a player cannot come back into the game until the 2 minutes have passed even in the case of a goal. -If a player receives a red card, he shall be sent off for the rest of the game (if given while playing then the team must play a player down). - If a player receives a red card, he will be suspended from playing the following match. - A player can still receive a yellow or red card even if he is on the sidelines. - Players will not be allowed to play under the influence of alcohol or recreational drugs. - All decisions are up to the referee of the game and must be respected by the players. - If a player receives two yellow cards in a game or a red card he will be suspended for the following game. In situations where a player starts a fight and/or fights back, both players will receive more severe punishment/suspension. Playoffs: -Only the top 2 teams from each group will qualify for the semis, which will be determined based on points > goal difference > goals for > goals against > Head to head > fair play - Please note that groups will have 4 teams each. -Playoff games will go to penalties if tied. - If a Playoff game is tied it will end 3 minutes earlier to allow for penalties. Forfeits: - If any team is 5+ minutes late they will be immediately forfeited. - If a team has less than 5 players on the field it will be considered a forfeit. - If a team is forfeited then the opposing team will automatically be granted a 3-0 win. Photo & Video Release agreement: By registering in this soccer league and agreeing to this form I grant permission to Super League Ottawa, the rights to my image, in video or photographs, and of the likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published, or distributed, and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this Photographic, audio, or video recordings material may be used for ANY USE which may include but is not limited to: • Website content; • Advertisement; • Online/Internet Videos; • Media; • News (Press); •Instagram, Facebook and/or YouTube; Players safety: - Dangerous play will NOT be tolerated. -There will be ZERO TOLERANCE for any inappropriate language / any aggression towards any of the players, referees, or organizers. If any violation occurs it will be up to the referee and the organizers to apply disciplinary penalties against the players involved such as (game suspension, suspension for the rest of the season, or banned) without any refund! - If a player engages in any aggressive behavior, it is the TEAM'S responsibility to stop him from carrying on with such behavior otherwise disciplinary penalties will be taken against the whole team.
